How to Manage Identity Theft
What is the ID Theft Affidavit?
It's a helpful standardized form. Once you fill it out, it provides a signed statement documenting the facts of your case. This can be submitted to credit reporting companies, your bank and creditors, and creditors for new accounts.
To request a copy of the ID Theft Affidavit form, call the FTC Identity Theft Hotline or go to www.consumer.gov/idtheft. The form requires:
- Your full name, address, and phone number
- Your birth date, Social Security number, and driver's license number
- A detailed description of what was stolen and how it was used
You also will need to attach:
- Your photo identification and proof of your address
- A copy of any police report you've filed
